Step 2: Location and Date

Enter the location of the test and the test event date in Step 2 of the form.

Step 2: School/System/Group Details appears after you have completed Step 1 and selected Next Step. All fields are required.

To complete Step 2:

1.    Enter the School/System/Group Name of your institution.

2.    Enter the street address (no P.O. boxes accepted.), the city, state, and  the Zip code of your location.

3.    If you are creating labels from a previous test administration, write the previous test administration order number on the Purchase Order # line. Otherwise, skip to the Test Date field.

4.    In the Test Date field, enter a date for the test using any month-day-year format (for example, 08-10-2018, 081918; slashes, dots, etc. are also acceptable).

5.    Click Next Step.
