A child's record must already exist in Data Manager in order for an assessment to be added to that record. Follow the procedure for Adding a Child Record if you need to add the child to the system before proceeding. To enter scores for a new complete assessment to a child's record:1. Click Search Child from the Child Administration menu and find the child's record (see Search for instructions). 2. Click the Test button in the row of the child's record. The Assessments page opens. 3. Click the New Complete Assessment button. The New Complete Assessment page opens. Instructions appear at the top of the page. Fields for recording test data appear at the bottom. 4. Select the Scoring Method, Raw Scores or Item Details. For raw scores, you will enter all information and totals on this page. For item-level details, you will enter domain, date, and examiner on this page, and then click the Next button to proceed to a detailed scoring page with individual assessment questions. Note: If you enter raw scores, you are responsible for the basal and ceiling for each score. Not all reports are available using this method. 5. Select a Norm Type, BDI-2 Norms or BDI-2 NU Norms. The NU norms are based on the most recent census updates available. Important: Use the same normative set for exit assessment as was used for entry assessment, whenever possible. 6. For the Domain list, click the check boxes to identify the domains that were evaluated during the assessment. Click the All check box to select all of the domains. 7. For Test Date, type the test date (mm/dd/yyyy) in the box corresponding to each domain that was assessed. To quickly add both domain and test date information: a. Click the calendar icon. b. Select the date from the pop-up calendar. c. Select the domain(s) assessed on the date selected. d. Click the Select and Continue button to close the calendar box and add your selections to the child record. 8. Identify the Examiner for each domain either by typing the examiner's name or clicking the examiner icon and selecting the name and corresponding domain(s) from the examiner selection box. Click Save and Continue to save your selections. 9. Enter the scores. Use the following guidelines as you enter information. – If you selected Raw Scores as your entry method, type the scores for each domain in the appropriate text box. When you finish entering the scores, click Save at the bottom of the page. Note: If the assessments were administered in Spanish, click the corresponding check boxes under the Spanish heading to indicate the procedure (Structured [S] or Interview/Observation [I/O]) used to administer the assessment for each domain. – If you selected item details as your entry method, click the Next button at the bottom of the domain list and select options to indicate the score and procedure for each item that was administered. When you finish entering items, click Done at the top of the page to proceed to the next selected domain. When all domains selected for testing/recording have been entered, you are returned to the assessment summary page. Raw scores for the domains have been added based on your item detail scores. – Use any additional features as needed. ○ Click the Program Note button to enter or view existing program notes for the assessment. ○ Click the Test Observations button to add validity information and/or notes about the child and the test session. ○ Click the Domain Totals button to view complete totals for each assessed domain. ○ For the item details entry mode only: 1 Click the Show Basal / Ceiling button to view the basal and ceiling item numbers for the current domain. 2 Click Note beside any individual item to open a small text box for entering additional information. 3 Click Clear beside any individual item to clear a previously entered score and/or procedure. 4 A Spanish Retest button appears when a child scores below a certain level, and other factors indicate a retest in Spanish could result in a substantially different outcome. After you have entered all the information for the assessment, the assessment page shows the age-equivalent (AE), percentile rank (PR), and standard scores (SS) for each domain. |
